Health Plan Eligibility:
  • Children in the care and custody of the Missouri Department of Social Services, including foster care and the custody of Division of Youth Services (DYS)
  • Children receiving adoption or legal guardianship subsidy
  • Youth who exited foster care after the age of 18 and have not turned 26

CASE MANAGEMENT
The Show Me Healthy Kids care management team will serve as the hub for accessing services and navigating benefits for eligible members. The hub consists of Care Managers (nurses and social workers), Care Coordinators Community Health Service Representatives, Foster Care Liaisons, and Transition Coordinators.  The hub teams are located across the state to provide support to School Nurses at the local level. ​Download info sheet.
 
COLLABORATION AND ACCESS TO SPECIAL SERVICES
Show Me Healthy Kids and School Nurses can work together on the following things:
  • Asthma health coaches (Asthma Waves program)
  • Lead Care management
  • Sickle Cell Care Management
  • Community resource referrals
  • Flu vaccination and scheduling EPSDT visits
  • Transportation for doctor appointments, lab tests, and any other non-emergency medical appointment
  • Pregnancy care management
  • General Pediatric Care Management
  • Caregiver Support including Caregiver Training and Caregiver Support Group Meetings
  • Behavior, Physical health, and social resources
  • Individualized Education Plan (IEP) coordination
  • Adolescence to Adulthood Program

  • Rewards for completing healthy behaviors
  • 24- hour Nurse Advice Line
  • After-school programs
  • Medication lock boxes
  • Sound machines
  • Health coaching for members with asthma, diabetes, depression, ADHD, and pediatric obesity

COMMUNITY OUTREACH
Staff from our Care Management team will be outreaching and have regular contact with discharge planners, social workers, residential facilities, adult members, legal guardians, case managers, and placement providers to coordinate care and case manage the member, especially during difficult times.

Show Me Healthy Kids is proud to provide a Community Resource Directory to help quickly and easily find resources for food, health, utility assistance, housing, job training programs. The Find Help website can be accessed by School Nurses and Show Me Healthy Kids members.
